Amber has been working with seniors and their families for 10 years to ensure they enjoy the best quality of life possible, through every stage of living. She began her career with seniors in the California Bay Area from 2009 to 2018, focusing on Stroke, Alzeheimers, Parkinsons, and other Neurological ailments during her healthcare career.
Amber is an Arizona native born in Phoenix, mother, and wife; she obtained her Bachelors degree in Business Management. This paved her way into the hospital organization where she had hands on experience in a rehab setting, focusing on Stroke and Neurological Traumas. Her focus was to navigate individuals and their families back into the community, with support of resources to make that transition possible.
Amber is devoted to consulting with seniors and their families as they move from living on their own to Assisted Living and Memory Care. Her dedication comes from her many years in Home Health Care and Hospice Services. The caring and supporting of her own father as he transitioned through the stages of aging; led Amber to want to help other seniors and their families within senior housing placement process.
Amber knows first-hand, the difficulties, anxiety, and stress that families face with these significant changes; and is willing to jump in to assist with that process.

Assisted Care Homes are considered private pay and are an out of pocket expense.
Medicare and Medicaid are not covered as a form of payment in Assisted Living Care homes.

Sunrise Senior Placements Services of Arizona (SSPSAZ), LLC; is a proud member of PASRS (Professional Association of Senior Referral Specialists) Central Arizona Chapter of the NPRA (National Placement & Referral Alliance).
Referral Fee Disclosure: The services provided to the client and their families is done so at no fee. SSPSAZ will offer resources and placement information at no fee to the client or the family. There will never be anything billed to the client, or owed to SSPSAZ for the services rendered by the client. Compensation made to SSPSAZ is done so by contract terms only between SSPSAZ and the facility, community, or residential care home for placements accepted. All terms of the placement fee are done so in writing and before any client is referred to the facility, community, or residential care home. Once a client is placed and SSPSAZ will bill the facility, community, or residential care home for the agreed upon contract fee.
Best Practices Disclosure: SSPSAZ will abide by all universal best practices set forth by PASRS. SSPSAZ will abide and honor all Medicare/ Medicaid policies at hospitals, skilled nursing facilities, home health and hospice companies regarding vendor visitation, gifting and compensation of referrals. SSPSAZ will not pay for or compensate employees of these companies for referrals. SSPSAZ will disclose to clients best practices and how agents are compensated. SSPSAZ will provide a referral fee disclosure to the client and the facility, community, or residential care home (property) at time of placement. In accordance with state laws no fee of any kind can be charged or accepted by SSPSAZ for clients placed under the ALTCS/Medicaid Program. SSPSAZ respects family involvement and choice, and will continue to respect the market field and always offer family choice. SSPSAZ will make every effort to arrange a personal interview with the client and involved family members prior to touring a property, and attended the client during the tour. SSPSAZ will review most recent AZDHS (Arizona Department of Health Services) surveys and share all findings with clients regarding property. SSPSAZ will not work with any property that has a policy to recover referral fees from the client. SSPSAZ requires all properties carry a one million dollar ($1,000,000.00) General Liability Insurance Policy. SSPSAZ has a duty to inform all clients of any uninsured properties/ communities. Upon Placement SSPSAZ will help to facilitate any move-in paperwork required by the AZDHS i.e.,Physician Orders, Physician signed medication list, and TB clearance. SSPSAZ will stay current in all local and state was regarding assisted living law with attention paid to content of residency agreements, referral fees, refund policies, behavioral placements, termination clauses, caregiver and manager trainings. SSPSAZ will not engage in the practice of moving or relocating a previously placed client for additional economic gain (known as "churning"). Any issues following a placement of a client or requests to relocate a client must be addressed directly by SSPSAZ with the family and facility, community, or residential care home. Any relocation must be initiated by the family and/or property after all parties have attempted resolution. SSPSAZ will partner with the property and the resident to resolve any and all questions regarding pricing, residency agreements, furnishings, searches provided and support services prior to or after the resident moves in to the property.
